Regardless of your insurance, if you would like to receive Occupational Therapy services I must have a referral or prescription from your Primary Care Physician.

• I am IN-NETWORK PROVIDER with TRICARE INSURANCE
• If you would like to work with me and have Tricare Insurance, you must ask your doctor for a OT REFERRAL to Shelby O'Connor / Adventure Therapy Services. Therapy Evaluation and Treatment can not start before this step.

I am out of network with these insurances: 

• Aetna
• BCBS depending on the plan*
• UHC depending on the plan*
• Kaiser

I CAN NOT SEE the following insurances at this time:

• Medicaid
• Medicare
• CHP
• Denver Health
• Humana
• Kaiser Medicaid
